PDF Form process
To access the Portable Document Format (PDF) version of our registration form, you mush have the Adobe Reader installed. If you do not have the free Adobe Reader you can download it.
You should download the file to fill out the file. Some browsers will open the PDF in a window, this will not save the information to a file. To download the file right click the pdf link and chose “save link as”
Once you have installed the reader and downloaded the file you will be able to open the file and complete the form. Once completed, click on the menu bar “File” --> “Save As”
(make sure to save it to a location that you can find it again)
use the existing file name but add your family name to it.
eg. “Family Camp Registration Smith Family.pdf”
email the completed form to registration@campcrossroads.com
(you can do this by creating a new email message and attaching the newly saved file)
